Ust a similar card for two purposes
1.)   Have the prospect you give the package to to complete the card and mail it to you for follow up and entering into a database of prospects.
2.)   Ask the prospect to give the card to a friend that may be interested in joining.  This can really expand a prospect database for future follow-up.

Avery Products manufactures pre preforated post-card sheets #3263.  These sheets can be used with the Avery Wizard that interfaces with Word 2000 to create your own layout.  Keep it as simple as possible yet get the data you deem necessary.
